Car health costs exceed the costs of smoking?

The American Public Health Association has published a report which puts the health costs of car dependency in the US at about $402 billion per year (which is a lot more than the cost of smoking, estimated at $196 billion per year in the USA.

When I cycle home and look at the lines of people stuck on the freeway, well, I feel good, perhaps even a bit superior. Is this why car drivers dislike cyclists?
